TFH sample survey

It is my first time ordering from TFH and I received a sample with a request to fill out the survey. Where is this survey located?

The survey link should be in your Payment Confirmation email that you get after you email your order, get the Order Received email and and then pay for it. That final Payment Confirmation email should have the link about half way down. I’m not sure if the URL changes from time to time. I imagine so and it’s just a basic webform on a google drive. It takes less than a minute to complete.
